Membership Application

To apply for membership in FPRA, click the Membership Application link below.

Print the form, fill it out completely, and return it to the FPRA Dick Pope/Polk County Chapter with a copy of your resúme and a check for the appropriate dues and processing fees.  You may also type into the form on your computer but you must then print it out.  The form (pdf) is not able to be saved to your computer.  Remember, should you elect to fill out the form on your computer, you must print it out before you close or you will loose the typed information.
